Swafford, who won his first PGA Tour title in the event in 2017 when it was known as the CareerBuilder Challenge, came from behind to win TheAmericanExpress title by two shots from fellow American Tom Hoge.
A final round eight-under par 64 contained an eagle, eight birdies and two bogeys on the Stadium Course at PGA West and saw Swafford end the week on 23-under par.
The 34-year-old Swafford started with a round of 70 before kicking into gear and posting 65, 66 and 64 to win the title.
It was his third victory in total on the PGA Tour having also won the Corales Puntacana Resort and Club Championship in 2020.
The victory moved Swafford up to 61st in the Official World Golf Rankings from 166th the previous week.
